When you refer a patient for follow-up care, we make certain that your referral is carried out: the appointment is made, the patient is reminded in timely fashion, if transportation is needed it’s arranged for, and most important, we follow up with all parties to be sure that the appointment was kept. We also make sure that any next steps are coordinated and that all of the patient’s care providers are kept in the loop.
